Can we control our future ? What if we can make plans for our life, what will your future like?
Miss F from Nanjing, China, had followed her plan of life, came to Stork at the age of 32, with a high AMH data, 3.81ng/ml. Been through 2 oocyte-pick up surgery, froze and stored 25 mature eggs.
Two years later, Miss F meet her Mr.Right and got married. Tried to have a baby for a year but life is not what she expected. Did a test of her AMH again, only 0.8 left. At age 35, she thawed the eggs started IVF treatment. 20 out 25 mature eggs survived, after insemination, culture for 6 days, we created 9 embryos, and 5 of them graded 5BB and the other 4 were BC grade. Implantation fails or success, it really depends on whether the chromosome is normal or not, even if the gradings are good.
Embryo gradings are just for reference, Miss F choose to do IVF 3.0+, 5 BB grading embryos were all biopsied and 3 of them sent for PGS test. Two out of three are with normal chromosome, one is with partial abnormalities. With younger age group comes with lower chromosome abnormalities, she is very lucky that she had chosen to freeze eggs at a young age. According to statistics, the embryos that created by the 32-year-old group can have almost 60% normal chromosome rate. These 3 embryos are suitable for implantation, form our experience, transfer 1 embryo with normal chromosome can achieve 60~70% pregnancy rate.
As Miss F lives overseas, she didn't want to fly over for ERA, so we transfer 1 normal embryo with the traditional format. Unfortunately, we failed. She did ERA at the next month, found that the WOI(window of implantation) is different than others, intake progesterone for 96 hours, which is 24hours earlier than our traditional format, that might be the main reason why we failed the first time.
Two months later, Miss F took progesterone for 96 hours and with a normal embryo, we performed precise implantation. After two weeks, her ßHCG data reached 10,567! Three more weeks later, we have a heartbeat. She is now a happy mother-to-be.
What were the factors of a perfect ending? the answer is ’IVF 3.0+’. Without PGS, how can we know whether the embryos are normal or not? Without ERA how can we know the precise WOI for each one? It is a huge waste if we transfer a normal embryo at the wrong time. Also if we are at the right time transferring an untested embryo, is just gambling. Is not like we can depend on appearance only. With PP-IVF ( personalized precision IVF), we can make it work.
Life is full of uncertainty, we can not control our future, but we can freeze our eggs for our future. About egg freezing, some might not understand how it works or near heard of it, worrying about it is not useable in the future, or the thaw-egg pregnancy rate. Afraid of missing put the optimal age to conceive a child, at the age 35-37, the cost-performance ratio is the best. It is proofed by science, after age 38, the quality of egg drop rapidly, which made the coat of doing IVF a lot higher compared to use frozen egg at age 35-37.

There are some women are infertile due to the ovulation function failure, common reason include congenital under-developed ovary( e.g Tunner syndrome) ; premature ovarian failure (e.g due to infection, surgery, immune system or medication treatment, cause the premature ovarian failure). And some women are carrier of chromosome disease, for example hemophilia, using donated Egg can really help to slove the problems.

Egg come from ?
Most
girl donating oocytes aim to help people, few of them are patients doing IVF,
end up getting more oocytes that she need, so donation can be an option. Miss Y
was doing IVF, but because of she suffered from Polycystic Ovary Syndrome(PCOS), end up getting more than 20 oocytes,

After
passing the blood test and checking the qualification with the government,
donor can start treatment from menstruation cycle day 3, using medication for
around 10 days, egg are becoming more mature. Throughout the procedure, we
monitor the growth of egg using ultrasound and blood tests. Around cycle
day 14, egg-pick-up surgery will be arranged, under anesthesia. The surgery
last 10-15 mins, after 1-2 hours rest, treatment is completed.
- Side effects
of egg donation

Very few people ,around
1%, suffer from Ovarian hyperstimulation syndrome( OHSS) after the surgery ,symptom
include : abdominal accounts, poor appetite, decreased urine, shortness of
breath, vomiting, etc.

the matching process, the priority will be the blood type. Then we will
according the criteria that you make for the donor, try out best to find a

- Can egg be frozen?
Positive. Vitrification is a common use technology at crypreservaion of oocytes and embryos. Vitrification is a cryopreservation technique that leads to a glass-like solidification. Oocyte, embryo and blastocyst freezing by vitrification method for cryopreservation have been used for many years beside sperms preservation. Moreover, the use of vitrification technology for ovarian tissue cryopreservation to freeze eggs offers such an elderly women who sometime find more difficulty in conceiving or in maintaining pregnancy till full term because of old age compared to relatively younger women who might get better chances to get a healthy pregnancy. Furthermore, vitrification helps cancer patients who are looking to preserve their fertility later on after completing their treatment.

In IVF, we’ve been discussing the ideal thickness of endometrium for implantation.
Besides, is the criteria the same for both fresh blastocysts and frozen-thaw blastocysts implantation? Will it be the same? We did not have many studies on the above issue.
This September(2018), Human Reproduction, a research team from Canada, using the database of local IVF treatment, analyzed over 40 thousand implantation cycles of both fresh blastocysts transfer(BT) and frozen-thaw blastocysts transfer (FBT).
A result had been found: Clinical pregnancy and live birth rates decline as the endometrial thickness decreases below 8 mm in BT and below 7 mm in FBT cycles.
As shown in the study,in the BT cycle, the pregnancy rate for endometrium thickness ≥8mm is 43.2%, the abortion rate is 22%, the live birth rate is 33.7%. Comparing to the statistics of thickness.
In FBT, the pregnancy rate of endometrium thickness≥7mm is 38.3%~38.4%, the abortion rate is 26~28.4%, the live birth rate is 27.4~28.4%. Comparing to the statistics of thickness.
To conclude, the indicator of endometrium thickness for implantation are as following:
Fresh blastocysts transfer: ≥8mm
Frozen-thaw blastocysts transfer: ≥7mm
With a good uterus environment, comes with a better clinical outcome. Good things are worth waiting for.

Do you know what eggs collection is?
Say simply, it is collecting good quality oocytes to freeze them. When the number of good oocytes is collected a certain number, starting to thaw these oocytes and then fertilize them.
What kind of situation that women need egg collection?
Let’s me tell you. We all know that ovarian function depends on women’s’ age. As the ages grow, ovarian function begins to decline. Also, the quality of oocytes is gradually getting worse.
Then how do we know our ovarian situation?
There is one important index called Anti-Mullerian hormone (AMH) which is produced by small growing follicles and granulosa cells in ovary we can refer to.
Based on this index, we learn that AMH can be represented as the ovarian situation.
As result, on condition that our AMH is low, we have to seize the chance to collect the good quality oocytes while our ages are not too old (under 38 years old).
*If our age is more than 38 years old, that is completely another thing. What We should do is Blastocyst Collection, not egg collection.

Besides AMH, age can also convey the important sign to us. How many mature oocytes are enough for women to have a baby?

In view of this index, we should go further to know how to distinguish which oocytes are mature.
At first, we judge the cumulus-oocyte complex initially from I to III. Then by using enzyme to remove the cumulus, we can evaluate oocyte morphology and choose the oocytes which are in stage MII to frozen it.
*Only oocytes which are in stage MII is valuable and can be fertilized.
For whom is under 38 years old, the best way to enhance the pregnancy rate effective is egg collection.
According to these two indexes which is related to individual ovarian function, we will apply the individualized controlled ovarian stimulation (iCOS) to design the fittest protocol for each customer.
Besides the individually tailored treatment, we have the most simple and effective process for a customer who far from overseas.
The first time you come to Taiwan is for filling the relevant document and having a necessary examination such as hysterosalpingogram (HSG), and blood drawn. Subsequently, you would bring the injection and medicine back to your home. However, you don’t need to worry. There is a consultant would teach you how to operate the injection and take medicine.
When the treatment starts, what you should do is having blood drawn and taking ultrasound on a specific day in your country for checking the development of follicular.
By checking the report with the consultant and doctor, the doctor would decide the date for oocytes retrieval.
The second time you come to Taiwan is for oocytes retrieval and having the rest of examination such as Hysteroscopy(H-scopy) and Cervical Bacteria Culture (3in 1).
For each eggs collection, you only need to come to Taiwan two times.

No distance is too far for the two loving hearts
If the love is true,
the distance is just the power of making people grow.
As the metro pulled out of the station, the man wrote "I love you" on the window. His girlfriend left on the platform, and still gazed into his eyes. It was always not easy to say goodbye for them, especially the following flashbacks after separation.
In the patients at our fertility center, some couples are living apart by necessity. Their husbands could be in the army, fisherman, or overseas businessman. The wives need to face the daily challenges bravely and maintain their home fully.
She was 37 years old and married her husband 7 years ago. They had no children yet. Before visiting us, she had tried IVF three times and undergone several embryo transfers at the other clinics. The ending of this journey seemed still unclear. The doctor arranged the hormone tests of AMH (as an index of ovarian reserves), Prolactin (PRL), and Thyroid stimulating hormone (TSH), to evaluate her body status.
Her PRL value was higher than the reference, suggesting that it might interferes the ovulation. Usually, the elevated PRL could be found in the women with breast feeding. However, the PRL could also increase in the women lack of sleep or under stress. The elevated PRL causes hypogonadism, possible anovulatory, and a decrease in menstruation.
The doctor gave her some medications for the hyperprolactinemia, and subsequent IVF treatment. With a declined AMH value as 1.63 ng/ml, the individualized controlled ovarian stimulation protocol was designed. Since her and her husband lived apart, the doctor planned the strategy of egg collection to fit their schedule. The capacity of female eggs are highly correlated with the maternal age. Therefore, the doctor recommends them to collect at least 15 eggs to increase the possibility of success pregnancy. If she would like to have two children, more eggs could be required.
Entering the IVF cycle again made her feel lonely and stressful. After work, she took the bus from Keelung to Hsinchu, and walked to our clinic. The commuting would take three hours every time. Thus she visited us late, and was always the last one at the clinic suite. The obvious exhaust and anxiety showed on her face. She was afraid that she could not catch the lastest bus up. We all knew that it was such a difficulty to her.
Five mature eggs were collected in the first cycle. In this egg retrieval, the doctor found some unsynchronized follicles with small size. Thus she went through the subsequent duoble-OPU to get the surplus eggs. Until she had 8 eggs, her and her husband decided to give a first try.

Perseverance and spirit, secret of all triumphs

They do not have good cards;
They do have the perseverance and spirit
and overcame the premature ovarian failure and azoospermia eventually.
A quote of Taiwanese Poet, Xi Murong, is that “all in rush written in the book of youth.” Youth is like a blooming flower, at the brightest and most beautiful stage of the life. What is your definition of youth?
I still remembered the first day I met her. She came to our clinic for the health screening before getting married and prepared for the pregnancy. The uterine condition and fallopian tubes seemed normal. Unexpectedly, the chocolate cysts were found in her ovaries, with a declined AMH value as 0.76 mg/ml. The ovarian reserve showed severely declined, and it was unusual at her age (~20+ years).
Chocolate cysts is the symptoms of endometriosis which endometrial tissue grows out of the uterus, and in the ovaries. The cysts contain the menstrual blood, chocolate-like liquid, enlarging over months with the menstruation. Thus she often felt pain during the periods, but did not know the actual reason until now.
It was not the entire news. The results of her fiancé 's semen analysis displayed as azoospermia, which means that no sperm could be found in the ejaculated semen.
With such low AMH value, many people would choose to take the donated egg program. However, using the embryo generated from both donors' sperms and eggs is forbidden according to the reproductive legislation in Taiwan. The diminished ovarian reserve and azoospermia became two difficulties in getting pregnancy. Considering her age was still young, the fertility specialist recommended her to take the strategy of oocyte accumulation from multiple IVF cycles and to use the sperm bank.
She agreed with this suggestion. Instead of a fixed stimulation protocol, her doctor designed an individualized protocol to adjust her ovarian function. In her first IVF cycle, four mature eggs were collected and it encouraged them to continue the other programs.
Then their wedding followed behind.
In the wedding vows, they promised each other,
"I will love you, hold you and honor you,
I will respect you, encourage you and cherish you,
In health and sickness,
Through sorrow and success,
For all the days of my life."
After becoming the husband and wife, they came to the clinic every month to continue the oocyte collection. It was just like running a marathon, and some accidents happened. Eventually, the egg collection took her almost 22 months.
The embryologists performed the artificial insemination using IMSI (6000x magnification) in her precious, frozen-thawed oocytes. The culture condition was controlled very well to grow the fertilized eggs into the blastocysts.
After transferring a blastocyst graded as 4BB, she finally got pregnant, and the beta-HCG level was 500 mIU/mL.
Youth is like a blooming flower, at the brightest and most beautiful stage of the life. Although the ovarian reserve was diminished, the quality seemed not being affected by the maternal age adversely yet. With her whole perseverance and spirit, they finally start a sweet family.
