A birthday gift giving back to the future!

A Birthday gift giving back to the future!

"I make that as a birthday gift to myself. Thanks to the advanced modern medical technology, I can freeze my fertility at this age and have a hold of my future."

I did not plan to have any children at first. I enjoy the time with myself and my freedom. That was me a few years ago. But people change, especially when you are at your most vulnerable moment when you are sick. Someone in the family got sick, everyone in the family is called up to help. That's when I realized you can never have enough family members.

This is the moment when my thought of freezing my eggs appears. Your child is not necessarily staying with you the whole life. But it is always nice knowing that you have a family that cares about you. Raising a child is not an easy task in 2020, as well as having pets or keeping myself warm.

The thought planted in my head for so long, it finally sprouted. I turned 39 this year, is the same age that my mother gave birth to me. Time is flying. I realize that I have to take some action! I studied a lot online and found Stork Fertility Center's website. They have detailed blog posts and graphics at a glance. Suddenly I apprehend that the quality of your eggs is tile with your age.

When I realized that I am competing with time, I started doing research comparing different hospitals and fertility centers. I finally chose Stork Fertility Center. They specialize in infertility and fertility preservation. Their treatment plan aimed at achieving "4- F". Fewer blood tests, fewer ultrasound exams, fewer injections, fewer clinic visits. That's a big attraction for me as I can not get away from work that often.

I make that as a birthday gift to myself. Thanks to the advanced modern medical technology, I can freeze my fertility at this age and have a hold of my future. Some might say that you might never use those eggs. Why bother to spend all that money on that? I think egg freezing is like time traveling. When you are planning for a family maybe five years later, you will be thankful that you made the right decision. Time is irreversible, no matter how much money you own. A word before is worth two behind.

Stork Fertility Center's team is very professional and helpful throughout the treatment. They helped me to arrange the time so that I won't have to take so many off from work. In the end, I only take one day off for surgery day. The blood tests, ultrasound, and clinic visits were all arranged at mine non-working hours. They have their application. It shows your itinerary daily by just one click on your phone. And the surgery took only 10-15 minutes. The operation room's ambiance is warming, not like any other hospital. You feel comfier with the cozy atmosphere and friendly staff.

Egg freezing is like insurance. You will feel safer after having one. This is probably the most meaningful birthday gift that I ever had.

Note :

1. Stork Fertility Center offers "4-F" treatment. Fewer blood tests, fewer ultrasound exams, fewer injections, and fewer clinic visits. The treatment is done with a minimum of two clinic visits. Just like our case above, taking only one day off from work! Before anyone knows, you already finished the treatment!

2. The case froze ten mature eggs at her first treatment.

Doctors suggest that the number of frozen mature eggs in each age group is different. It is related to age. With higher age, comes with a higher chromosome abnormal rate. Egg freeze treatment is suggested before age 35. The pregnancy rate and live-birth rate are relatively better.

3.The recommended number of mature eggs for cryopreservation for bringing a baby home at different ages is as follows:

AGE

35 or below

35~38

39~40

>40

Number of mature eggs

10#

15#

20#

>20#

Sperm Freezing

What is sperm freezing?

Sperm is one of the germ cells of the human body, which can be frozen and stored for years.
What is the purpose of sperm freezing? Mostly for later use for reproductive reasons, such as IVF( In-vitro fertilization) or IUI( intrauterine insemination). As well as sperm donation.

How?

Sperm freezing is a procedure of collecting, analysing, selecting and freezing.
Normally, sperm collection is through masturbation, or for patients with ejaculatory dysfunction, Doctors might perform surgery to collect sperms. For example, MESA( sperm aspiration from the epididymis by means of microsurgery), TESA( sperm extraction from the testicles by means of a biopsy).
Patients can choose to collect a sample in either their home or in our clinic.
As we mentioned before, the stored sperm sample can be used for IVF and IUI later on. But for patients collected sample through MESA/TESA, IVF Is highly recommended. Aimed to increase the successful pregnancy rate with limited sample size.
Before the sperm cryopreservation, there is some preparation to do:
-Blood test for infection screening, eg. HIV, RPR, hepatitis.
-If you have a previous semen analysis report, please provide to us for reference.

Vitrification
The technology we are using to freeze the sperm now is the vitrification. Compared to the traditional method, this new technology uses less time to finish the freeze, lowering the harm that can possibly be made during the procedure.
The sperm sample will be analysed and selected immediately. Approximately 1-2 hours after, the sample will be frozen for storage.
For the post thawed survival rate, it’s around 20-50%, depends on the quality of the sperm sample.

Stork Application

You can find all the blood test results ( HIV, RPR, hepatitis) in our self developed app. As well as the semen analysis report. With the list of sperm samples that had been frozen.

Who should consider freezing sperm?

Freezing germ cells allow us to preserve our fertility. Using the frozen cell at a later date, age is no longer an obstacle for conceiving an offspring.
Common reasons to choose to freeze sperm:
Advancing age
Cancer or other medical reasons
Transgender patients
Career and lifestyle choices, such as studying aboard for higher educational activities

The Egg Retrieval Surgery

The egg-retrieval process for oocyte cryopreservation is the same as that for in vitro fertilization. This includes 2 – 4 weeks of hormone injections and hormonal contraception in order to stop ovulation, followed by more hormone injections to stimulate ovaries and ripen multiple eggs. When the eggs matured, additional hormone is given and the eggs are removed with an ultrasound-guided needle through the vagina. The procedure is conducted under anesthesia.

The word "surgery" might seems scary, but actually egg-retrieval surgery only takes 15 to 20 minutes and it's painless!

HOW IT WORKS?

The doctor will use ultrasound to observe your ovaries, which will look like a cluster of grape by now. Your doctor will penetrate a thin needle through vaginal wall and gently reach out to one side of ovaries and extract the eggs. After finish one side, the other side will be done as well. These eggs will be collect into test tubes with your name and number on, then will be handed over to the embryologist who is an expert of oocyte cryopreservation. The embryologist will exam and count your eggs.

DURING SURGERY, YOU WON'T FEEL A THING

The anesthesiologist will put you under anesthesia by intravenous injection. This will make you fall asleep through the whole procedure, so you won't feel a thing, you might even have a sweet dream!

AFTER THE SURGERY

When complete the retrieval process, the doctor will exam your vaginal wall. There might be a little bit of bleeding so doctor might apply pressure on the wound site. The nurse will check on you by patting your shoulder and calling your name. You will rest for 60 to 90 minutes in the recovery area, where the nurse will keep on monitor your status. When you wake up from anesthesia, we will teach you how to take care your wound and how to take some medication for precaution. Finally, you can enjoy a nice and free meal at our cafe.

After egg retrieval, you may feel some soreness, bloated or some cramping in abdomen area, which could last few days. We suggest you go straight home by taxi or escort by a friend (or family member) and take the rest of the day off. Just put your work and responsibility aside temporarily and chilling. It's the perfect time for you to fall in love with oppa in Korea drama or to catch up on Netflix series!

YOU CAN SEE YOUR EGGS ON-LINE!

Before you leave our center, we will give you a set of account and password of your on-line medical record, where you will have your own "oocytes deposit book". The next day of retrieval, the embryologist will let you know the condition of your eggs and how many mature eggs were ultimately frozen. You can see photos of your oocytes taken by embryologist at your on-line oocytes deposit book anywhere and anytime!

Perseverance and spirit, secret of all triumphs

Josh Billings said,

“life consists not in holding good cards but in playing those you hold well.”

They do not have good cards;

They do have the perseverance and spirit

and overcame the premature ovarian failure and azoospermia eventually.

A quote of Taiwanese Poet, Xi Murong, is that “all in rush written in the book of youth.” Youth is like a blooming flower, at the brightest and most beautiful stage of the life. What is your definition of youth?

I still remembered the first day I met her. She came to our clinic for the health screening before getting married and prepared for the pregnancy. The uterine condition and fallopian tubes seemed normal. Unexpectedly, the chocolate cysts were found in her ovaries, with a declined AMH value as 0.76 mg/ml. The ovarian reserve showed severely declined, and it was unusual at her age (~20+ years).

Chocolate cysts is the symptoms of endometriosis which endometrial tissue grows out of the uterus, and in the ovaries. The cysts contain the menstrual blood, chocolate-like liquid, enlarging over months with the menstruation. Thus she often felt pain during the periods, but did not know the actual reason until now.

It was not the entire news. The results of her fiancé 's semen analysis displayed as azoospermia, which means that no sperm could be found in the ejaculated semen.

With such low AMH value, many people would choose to take the donated egg program. However, using the embryo generated from both donors' sperms and eggs is forbidden according to the reproductive legislation in Taiwan. The diminished ovarian reserve and azoospermia became two difficulties in getting pregnancy. Considering her age was still young, the fertility specialist recommended her to take the strategy of oocyte accumulation from multiple IVF cycles and to use the sperm bank.

She agreed with this suggestion. Instead of a fixed stimulation protocol, her doctor designed an individualized protocol to adjust her ovarian function. In her first IVF cycle, four mature eggs were collected and it encouraged them to continue the other programs.

Then their wedding followed behind.

In the wedding vows, they promised each other,

"I will love you, hold you and honor you,

I will respect you, encourage you and cherish you,

In health and sickness,

Through sorrow and success,

For all the days of my life."

After becoming the husband and wife, they came to the clinic every month to continue the oocyte collection. It was just like running a marathon, and some accidents happened. Eventually, the egg collection took her almost 22 months.

The embryologists performed the artificial insemination using IMSI (6000x magnification) in her precious, frozen-thawed oocytes. The culture condition was controlled very well to grow the fertilized eggs into the blastocysts.

After transferring a blastocyst graded as 4BB, she finally got pregnant, and the beta-HCG level was 500 mIU/mL.

Youth is like a blooming flower, at the brightest and most beautiful stage of the life. Although the ovarian reserve was diminished, the quality seemed not being affected by the maternal age adversely yet. With her whole perseverance and spirit, they finally start a sweet family.

Her clinical outcomes were showed as below,
