IVF journey x Eggs Collection

Do you know what eggs collection is?

Say simply, it is collecting good quality oocytes to freeze them. When the number of good oocytes is collected a certain number, starting to thaw these oocytes and then fertilize them.
What kind of situation that women need egg collection?
Let’s me tell you. We all know that ovarian function depends on women’s’ age. As the ages grow, ovarian function begins to decline. Also, the quality of oocytes is gradually getting worse.
Then how do we know our ovarian situation?

There is one important index called Anti-Mullerian hormone (AMH) which is produced by small growing follicles and granulosa cells in ovary we can refer to.

Based on this index, we learn that AMH can be represented as the ovarian situation.
As result, on condition that our AMH is low, we have to seize the chance to collect the good quality oocytes while our ages are not too old (under 38 years old).
*If our age is more than 38 years old, that is completely another thing. What We should do is Blastocyst Collection, not egg collection.

Besides AMH, age can also convey the important sign to us. How many mature oocytes are enough for women to have a baby?
There is an index as below:

In view of this index, we should go further to know how to distinguish which oocytes are mature.
At first, we judge the cumulus-oocyte complex initially from I to III. Then by using enzyme to remove the cumulus, we can evaluate oocyte morphology and choose the oocytes which are in stage MII to frozen it.
*Only oocytes which are in stage MII is valuable and can be fertilized.

For whom is under 38 years old, the best way to enhance the pregnancy rate effective is egg collection.
According to these two indexes which is related to individual ovarian function, we will apply the individualized controlled ovarian stimulation (iCOS) to design the fittest protocol for each customer.
Besides the individually tailored treatment, we have the most simple and effective process for a customer who far from overseas.

The first time you come to Taiwan is for filling the relevant document and having a necessary examination such as hysterosalpingogram (HSG), and blood drawn. Subsequently, you would bring the injection and medicine back to your home. However, you don’t need to worry. There is a consultant would teach you how to operate the injection and take medicine.
When the treatment starts, what you should do is having blood drawn and taking ultrasound on a specific day in your country for checking the development of follicular.
By checking the report with the consultant and doctor, the doctor would decide the date for oocytes retrieval.
The second time you come to Taiwan is for oocytes retrieval and having the rest of examination such as Hysteroscopy(H-scopy) and Cervical Bacteria Culture (3in 1).
For each eggs collection, you only need to come to Taiwan two times.

Does the mild stimulation improve embryo euploid rate in diminished ovarian reserve women?

The psychological and financial burden of IVF increases while the ovarian function declines.

If the frequency of injection decreases, would the patients still have enough embryos for transfer?

Women with diminished ovarian reserve (DOR, which was defined as Anti-mullerian hormone < 2 ng/ml and Antral follicle count ≦ 5 here), are confronted with insufficient ovarian reserve. Although controlled ovarian stimulation (COS) is an efficient treatment for infertility, it involves costly injections for the purpose of retrieving more oocytes during cycle. In order to evaluate the efficiency of different ovarian stimulation protocols in DOR patients, the chromosome constitution identified by using NGS platform was used as a parameter for assessing the embryo quality.

During January 2015 to June 2016, 52 patients (mean age: 38.1 years) with AMH < 2 ng/ml and AFC≦5, were randomly assigned to undergo strong stimulation (long-acting r-FSH and short-acting r-FSH with daily injections), or mild stimulation (short-acting r-FSH with every-other-day injections), or mini stimulation (clomiphene citrate only or natural cycle). A total of 139 embryos from 86 IVF/PGS cycles were biopsied on culturing day 5 or 6, and then analyzed on NGS platform (Miseq®).

Compared with strong stimulation group, mild and mini stimulation groups result in fewer retrieved oocytes per cycle. (Strong:6.1# Ovum; Mild:4# Ova; Mini:1.9# Ova). Moreover, studied patients with advanced maternal age (≧38ys) undergoing mini stimulation showed a tendency to possess more embryos available for biopsy in total retrieved oocytes (Mini: 64.4% v.s. Strong: 35.9%). And the euploid rate of mild and mini stimulation groups were higher than that of the strong stimulation group (Mild plus Mini: 27.6% v.s. Strong: 20.8%). It is known that the euploid rate decreased with increasing maternal age, and mini stimulation seems to have the potential to improve the euploid rate in the patients with DOR.

The preliminary data suggests that different ovarian stimulation strategies could affect the number of retrieved oocytes and chromosomal abnormality of embryos. A friendly ovarian stimulation strategy should concern both the efficiency and the feeling for patients.

Eight things to make IVF fail

What are the causes of IVF failure? Young patients usually get their IVF babies after 1~2 cycles. In the patients over 38 years or with low ovarian reserve, multiple IVF cycles are always necessary. Eight things are common in the patients over 38 years or AMH < 2 ng/ml.

Eight things to make IVF fail
Poor response
The follicles cannot be stimulated by external rFSH. It may result in the follicle atresia or growth arrest.

Premature ovulation
Ovulation in IVF generally occurs 36-40 hours after the triggering. Premature ovulation during IVF results in the cancellation of oocyte retrieval.

Empty follicle
Mostly one follicle contains one oocyte. It is found that there is no oocyte in a follicle after the retrieval.

Low oocyte yield
Only mature oocyte (MII) can be fertilized. Nearly 70% of the retrieval oocytes can be used for the following fertilization in the patients under 35 years; in those over 40 years, the yield is dropped down to 50% or even lower.

Low fertilization rate
The general fertilization rate (2PN) in IVF is around 70%, but it can be lower or complete fertilization failure.

Abnormal fertilization
Two pronuclear zygote (2PN) is observed after a normal fertilization. An abnormal fertilization happens while three pronuclei (3PN) or mutiple pronuclei are observed.

Developmental arrest
Around 95% of 2PN zygotes grow into the cleavage stage. Developmental arrest could happen at any time point during embryo mitosis, e.g. one-cell arrest, 2-4 cells arrest, 6-8 cells arrest, arrest at compaction stage...etc.

Low blastocyst formation rate
After five days of culturing, an embryo undergoes cell proliferation and differentiation to become a blastocyst. No blastocyst formed or no blastocyst with good morphology formed at the end of culturing happened in IVF sometimes.

What can we do to the eight things?
Poor response
Individualizing stimulation protocol by adjusting with maternal age is a method, such as mild stimulation, natural cycle, or multiple retrievals in a IVF cycle.

Premature ovulation
Individualizing the time of triggering and retrieval by adjusting with maternal age, follicle size, and total follicle count. Non-steroid anti-inflammatory drugs (NSAIDs), which was reported to prevent premature ovulation in the previous data, can be applied 8~14 hours before the retrieval as well. For an example, the trigger shot may be injected while the follicle reaches to 17 mm in diameter to the patients under 40 years, but it may be injected while the follicle reaches to 15~17 mm in diameter to those over 40 years to prevent the premature ovulation.

Empty follicle
It cannot be avoid if the empty follicle forms naturally.

Low oocyte yield
The patient can undergo multiple IVF cycles to harvest available oocytes (oocyte accumulation).

Low fertilization rate
The quality of sperm and oocyte could be the reasons. The ICSI/IMSI can be applied to select the sperm with better morphology. Of the quality of oocyte, it cannot be solved due to the effect of maternal age.

Abnormal fertilization
If it resulted in abnormal fertilization (3PN or multiple PNs) while the ICSI/IMSI had been applied, the quality of oocyte could be the main reason. Oocyte accumulation by multiple IVFs is the only solution.

Developmental arrest and Low blastocyst formation rate
Similarly, developmental arrest or no available blastocyst formed during culturing occurs spontaneously and cannot be avoid. Embryo accumulation by multiple IVFs is the only solution.

It is popular to donate the eggs during summer vacation

casual, cute, female

In Taiwan, the compensation fee for egg donors is NTD 99,000 (USD 3,056). By this considerable compensation fee, some donors were able to join the study-tour, travel abroad, or open their own stores.

According to the local news, the egg donation became more popular in the young females recently. Due to the network convenience, more and more people got the information of egg donation easily, and the enrollment is to double during the summer vacation. Amy just completed her donation program, and she immediately recommended the program to her twin sister. She believed that the donated eggs could help the infertile couple to have their own babies. In this program, she has not only checked her physical status, especially the ovarian function, but also got more understandings to the IVF.

However, the risk of egg donation must be concerned before starting the program. The ovarian hyperstiumulation syndrome (OHSS) is most common in the young females undergoing the controlled ovarian stimulation (COS). An experienced specialist, a stable system of follow-up care, and a reputable fertility center are prior consideration to donate your eggs.

How to avoid the ovarian hyperstimulation?

1. Use GnRH anagonist as the trigger, such as Decapetyl

2. Drink lots of water to dilute the hormone in the body (>2000 ml per day)

3. Take more rest and no strenuous exercise

4. Low-salt and high-protein eating plan

5. Contact with the center whenever you feel uncomfortable

6. Follow-up exams at the center (CBC, ultrasound...)
