[Patient Testimonial] A Long Journey, A Solid Friendship-Sperm Donation Program

At the end of 2014, when I was planning to go watch the fireworks show at Hong Kong’s Victoria Harbour to celebre the new year, I got Maddy’s message. In this Message, she told me that she delivered a healthy baby. Therefore, I suddenly got an idea which is going to visit Maddy’s home and to see her baby.

How did I meet Maddy? The Answer is an international phone call from Hong Kong. In this phone call, I discovered why Maddy and her husband chose to go Taiwan for IVF treatment (In vitro fertilisation). Her husband has Azoospermia( which is the medical condition of a man whose semen contains no sperm. ) due to him getting mumps when he was a child. They wanted to do a sperm donation program. Then I started to help Maddy to figure out how to reduce the cost for doing IVF in the Taiwan, and plan a personal treatment schedule for Maddy(she just need to come to Taiwan only few times to finish this whole IVF treatment).

I had very hard time to understand Cantonese when I spoke with Maddy’s husband when we first met. Fortunately, Maddy translated what her husband said perfectly in Mandarin to me. It really helped us when we got into our first OPU (oocyte pick up) and BT(Blastocyst Transfer). However, the BT did not at first bring us good news. Maddy wasn’t pregnant, but she still kept her positive attitude and actively continued the next OPU.(because Maddy wants two babies and she only had one Embryo left at that time)

During Maddy’s second time treatment, she told me that her mom will come to Taiwan with her ,and they will stay in Taiwan for one month. Apparently, housing will be a problem for Maddy because there are few affordable Hotels in Taiwan for one month. Thus, I help her to find a short term rental. Eventually we became neighbors, with Maddy renting an apartment near my apartment.

After I met Maddy’ mom, I immediately know where Maddy got these positive attitude and optimistic personality. Maddy’s mom not only helped cheer Maddy up everyday, but also took care Maddy’s daily meals with her wonderful cooking(I was invited to have dinner with them couple times). A few weeks later, The outcome of this time’s OPU was great, we got 12 eggs. 8 out of 12 are successfully fertilized in the same day. Finally, Maddy got 6 good embryo. 15 days later, Maddy is pregnant. When Maddy was on her way home(Hongkong), I saw her husband come pick her up back. I know this baby is the best souvenir to them in this long trip.

First Visit to Taiwan
**Mainly for concerning examination and sperm freezing, staying for only 1 days during MC Day 7-13

I. Meeting with the Doctor.
II. Complete and notarize Sperm Donation Program consent, IVF consent, Kinship Form and regarding documents.
***Two identifications of recipient couple are required (ID card and Passport)
III. Recommended Examinations as below.

·             Wife's Examination 
1.       HSG (hysterosalpingogram)
2.       Blood test of Autoimmune and Hormone

IV. Decide whether PGS (Preimplantation Genetic Screening) or not
V. Discuss your IVF treatment with the Doctor and when to start ovarian stimulation.
VI. Gain the personal IVF protocol and bring the medication back home.


Second Visit to Taiwan
***Mainly for follicle growth monitoring and eggs retrieval surgery, staying for 5-7 days during MC Day 7-9

I. Follicle growth monitoring 
II. Eggs retrieval surgery

2.    Cervix Culture Test
IV. Eggs freezing (Eggs collection)
V. Nsemination and Blastocyst culture (For Embryos Collection)


Third Visit to Taiwan
***Mainly for transfer, staying for 3-5 days during MC Day 18-20

Fresh embryo transfer cycle
Synchronize the mense cycle with the donor by taking hormone pills (2 to 3 months required).
l    Donor starts the stimulation procedure, and recipient starts taking pills for endometrium preparation.
(If you used our oocyte donor bank, the donor has completed oocyte retrieval.)
l   Recipient follows up endometrium thickness by ultrasound.
l   Stork Fertility Center will give notification if the date for donor’s egg retrieval is determined. (2 days before the egg retrieval)
l   On the date of egg retrieval, husband collects sperm sample in Stork Fertility Center. (Or using the frozen sperm for fertilization).
l   Fertilization takes place, embryo culture to day5-6.
l   Fresh embryo transfer, around TWD 50,000-60,000 per cycle.

Frozen embryo transfer cycle
l   Donor starts the stimulation procedure. (If you used our oocyte donor bank, the donor has completed oocyte retrieval.)
l    Stork Fertility Center will give notification if the date for donor’s egg retrieval is determined. (2 days before the egg retrieval)
l    On the date of egg retrieval, husband collects sperm sample in Stork Fertility Center. (Or using the frozen sperm for fertilization).
l    Fertilization takes place, embryo culture to day5. And freeze all the resulting embryos.
l    Frozen embryo transfer will be arranged according to recipient’s next cycle, around TWD 50,000-60,000 per cycle.

Traps from delicious foods? Eat right and have better sperm




Do you like wearing a tight pants or putting your laptop on your thighs? These habits may affect the quality of your sperm. Besides, five types of drinks/foods may harm your reproductive system. Let’s check and keep in mind!

According to <<Epicurious>>, if the quality of your sperm is not good, it may affect your family plan. To maintain the good quality of sperm, the uptakes of following drinks/foods must be controlled.

 1. Alcohol
Alcohols dehydrates the water in our bodies, and increases the risk of obesity, and reduces the mobility of sperms. We suggest not drinking alcohol more than once per week, and therefore the bodies can metabolize the alcohol. But if you have severe reproductive problem, it is better to quit alcohol.






2. Processed meat products
Bacon, ham and burger are delicious, but these processed meat products usually contain high proportion of saturated fat and hormone residues, which harm the quality of sperm. If you like meat, have the “fresh” one.







3. Sweet Drinks
Bubble milk tea or other sweet drinks contain high proportion of sugar stimulating pancreas to release more insulin, and they increases the risk of obesity. Sweet drinks reduce both the quantity and quality of sperm. It’s better for us to drink water.






4. High-fat dairy products
Milk, cheese and other dairy products can provide protein, calcium and other nutrients. But the high fat contained in these products may harm male’s reproductive system. Also, pesticide or hormone residues should be concerned in some dairy products.





5. Fruits with pesticide residues
Some of the fruits or vegetables may have pesticide residues, especially apples, grapes, strawberries, spinach, cucumbers, etc. We suggest choosing organic fruits and vegetables to decrease the risk.





Stay away from bad habits and unhealthy foods/drinks. Get your sperm good!

What should we know about male infertility



Like the infertility issues in females, 
male fertility correlates with the age, lifestyle, and individual background. 
If you are aware of these factors, 
you may know more about your own body state.


With the dramatic change in recent society, people tend to start family late. We usually choose to pursue career first, and put starting our own family at the second priority. This decision made more people encounter the infertility problems at their late 30s and 40s of age. More and more studies demonstrated that the quality of oocyte declines with increasing female age. Thus oocyte cryopreservation for the social reason becomes more popular. How about the men? Several studies also found that the mean age of fathering men showed increased since 1993 to date. Meanwhile, the percentage of male factor in the entire population with fertility problem is increasing as well. The following 4 questions are frequently asked by our male cases,





1. Should semen analysis be routinely clinical testing?


Reproductive potential gradually declines with advanced paternal age, but the declining speed is not as fast as maternal ovaries. The semen analysis is the most common method to check the general sperm quality. Some studies indicated that there is significant decrease in semen volume, sperm concentration, sperm motility and sperm morphology among older men. The pregnancy rate in the men at 40s displayed 10% down than that in the men at 30s. Thus the males with advanced age and abnormal semen parameters have higher risk for the infertility issue. Should the semen analysis be routine testing? Maybe NOT. Only the couple who has failed to conceive for ≧1 year is recommended.






2. Should PGS (preimplantation genetic screening) be performed in the men at advanced age? 


The rate of aneuploid embryo is higher in women at advanced age (≧36 years old), and thus PGS becomes an option for them to choose the embryo with normal chromosomal dosage. Is it possible that the aneuploid embryo derived from the sperm with advanced paternal age ? Hassold et al. demonstrated that one of the cause for trisomy 21 is paternal chromosome nondisjunction, and it accounted for 20% of the entire population with trisomy 21. A more-recently scientific article showed that significantly increasing aneuploid rate in embryos from the older men and men with oligozoospermia. It appears that suboptimal sperm can be a source of embryo aneuploidy. However, the correlation of paternal age and embryo aneuploidy is not as strong as that of maternal age. For those with partner at advanced maternal age, or with the recurrent miscarriage history or family inherited disease, PGS/PGD is recommended. Generally, the paternal age is not the sole indication for PGS.





3. What should the older father know?

Several reports showed the children with older fathers are more likely to develop certain pathologies, such as the schizophrenia. The rate of autism spectrum disorders slightly increases as well. For the father themselves, some medical comorbidities do affect the fertility, like heart disease and chronic medicines of hypertension. It could lead to erectile dysfunction and benign prostatic hypertrophy.




4. Should males cryopreserve their sperm for social reason?


The sperm motility could be halved after cryopreservation. For men with known fertile issue, cryopresevation may induce sperm DNA damage. Since the correlation between fertility and male age is not as strong as the female age, there is no need to cryopreserve sperm for the social reason, and the state of frozen sperm is not comparable to the fresh sperm.

Beyond examinations, healthy diet and lifestyle do help the fertility. It's not necessary to cryopreserve sperm at young age. If the body stays good, the quality of sperm may remain for a long time.

Perseverance and spirit, secret of all triumphs


Josh Billings said,

“life consists not in holding good cards but in playing those you hold well.” 

They do not have good cards;

They do have the perseverance and spirit

and overcame the premature ovarian failure and azoospermia eventually.




A quote of Taiwanese Poet, Xi Murong, is that “all in rush written in the book of youth.” Youth is like a blooming flower, at the brightest and most beautiful stage of the life. What is your definition of youth?

I still remembered the first day I met her. She came to our clinic for the health screening before getting married and prepared for the pregnancy. The uterine condition and fallopian tubes seemed normal. Unexpectedly, the chocolate cysts were found in her ovaries, with a declined AMH value as 0.76 mg/ml. The ovarian reserve showed severely declined, and it was unusual at her age (~20+ years).



Chocolate cysts is the symptoms of endometriosis which endometrial tissue grows out of the uterus, and in the ovaries. The cysts contain the menstrual blood, chocolate-like liquid, enlarging over months with the menstruation. Thus she often felt pain during the periods, but did not know the actual reason until now.


It was not the entire news. The results of her fiancé 's semen analysis displayed as azoospermia, which means that no sperm could be found in the ejaculated semen.

With such low AMH value, many people would choose to take the donated egg program. However, using the embryo generated from both donors' sperms and eggs is forbidden according to the reproductive legislation in Taiwan. The diminished ovarian reserve and azoospermia became two difficulties in getting pregnancy. Considering her age was still young, the fertility specialist recommended her to take the strategy of oocyte accumulation from multiple IVF cycles and to use the sperm bank.



She agreed with this suggestion. Instead of a fixed stimulation protocol, her doctor designed an individualized protocol to adjust her ovarian function. In her first IVF cycle, four mature eggs were collected and it encouraged them to continue the other programs.

Then their wedding followed behind.

In the wedding vows, they promised each other,

"I will love you, hold you and honor you,

I will respect you, encourage you and cherish you,

In health and sickness,

Through sorrow and success,

For all the days of my life." 




After becoming the husband and wife, they came to the clinic every month to continue the oocyte collection. It was just like running a marathon, and some accidents happened. Eventually, the egg collection took her almost 22 months.



The embryologists performed the artificial insemination using IMSI (6000x magnification) in her precious, frozen-thawed oocytes. The culture condition was controlled very well to grow the fertilized eggs into the blastocysts.

After transferring a blastocyst graded as 4BB, she finally got pregnant, and the beta-HCG level was 500 mIU/mL.


Youth is like a blooming flower, at the brightest and most beautiful stage of the life. Although the ovarian reserve was diminished, the quality seemed not being affected by the maternal age adversely yet. With her whole perseverance and spirit, they finally start a sweet family.
