I did not plan to have any children at first. I enjoy the time with myself and my freedom. That was me a few years ago. But people change, especially when you are at your most vulnerable moment when you are sick. Someone in the family got sick, everyone in the family is called up to help. That's when I realized you can never have enough family members.

The thought planted in my head for so long, it finally sprouted. I turned 39 this year, is the same age that my mother gave birth to me. Time is flying. I realize that I have to take some action! Advance examinations for IVF at 2020

EMMA and ALICE



What is EMMA (Endometrial Microbiome Metagenomic Analysis)?

The test is aimed to analyse the endometrial microbiome to help identify abnormalities associated with a poor reproductive prognosis.

 EMMA is a molecular tool used to determine whether the uterine microbial environment is optimal for endometrial health. This molecular method is based on detecting and measuring the amount of bacterial DNA present in the endometrial sample. 

Therefore, EMMA helps to determine when the endometrium presents a physiological bacterial flora. Lactobacilli accounts for less than 90% of the microbiotal in the endometrium which will lead to significantly decrease in pregnancy rate.

If the result comes back indicated “ ABNORMAL ENDOMETRIAL MICROBIOME” , doctors can provide appropriate probiotics suggested by the report for the patient to use in order to create a better endometrium environment for later implantation. 

Why use EMMA test?

  • EMMA uses the latest Next Generation Sequencing (NGS) technology to provide endometrial microbiome information by analysing the complete profile of the bacteria present in the tissue

  • EMMA can determine the percentage of Lactobacillus present in the endometrium; low proportions of Lactobacillus are associated with lower pregnancy rate. 

  • EMMA includes the ALICE test, so it indicates the possible presence of bacteria that can cause chronic endometritis, further other pathogenic bacteria.

  • If the endometrium is non-Lactobacillus dominated, the report will suggest the adequate treatment for each patient

  • EMMA will determine whether the uterine microbial environment is optimal or not for embryo implantation

Who should use the EMMA?

  •  Patients with repeated implantation failure (RIF)

  • EMMA can be beneficial for any patient wishing to conceive, by assessing the microbiological environment that the embryo will encounter at implantation

How is EMMA done?

Doctors will use pipelle endometrial suction curette to go into the uterus through vagina, acquire a small amount of endometrium samples. The whole procedure takes around 30 seconds to 1 minute. 

 

What is ALICE (Analysis of Infectious Chronic Endometritis)?

Chronic endometritis(CE)will lead to endometritis which is one of the reasons for infertility and under most circumstances patients do not appear with obvious symptoms.ALICE is  a test that detects pathogenic bacteria and recommends adequate treatment. In traditional IVF, doctors can not diagnose the bacterias  specifically, and can not prescribe the appropriate treatment. 

ALICE is a diagnostic test to detect and quantify the most common 8 pathogenic bacteria causing chronic endometritis, recommending appropriate antibiotic and probiotic treatment

  • -Enterobacteria (e.g. Escherichia, Klebsiella)

  • -Chlamydia

  • -Mycoplasma

  • -Neisseria

  • -Ureaplasma

  • -Enterococcus

  • -Streptococcus

  • -Staphylococcus

FAQ

  • Will frozen eggs be used?

There are not many things that can surpass the future. Egg freezing is one of them. Some people worry about “won’t use it”. This depends on “age” and “whether you want to get married”. If there is no stable relationship between the ages of 35 and 37, it means that the chance of marriage is not high at the age of 38 to 40. At this time, egg freezing is the right choice and the CP value is the highest. As empirical medicine shows that women’s egg quality declines rapidly at the age of 38, the cost doubles of IVF.

  • Do I have to get married to get pregnant with frozen eggs?

Yes, according to Taiwan’s Artificial Reproduction Law, a legal marriage relationship is required to implement artificial reproduction technology. Or you may choose to export your eggs abroad for related treatment services in the future.

  • How long can an egg be frozen?

Frozen eggs use vitrification technology to store eggs in a stable environment of liquid nitrogen at -196°C for a long time. Many studies have found that vitrification causes much less damage than slow freezing and has a survival rate of more than 90%. Therefore, there is no shelf life on the technical level.


  • Will an ovulation injection lead to earlier menopause?

No. The number of female eggs is determined at the moment of birth. There are already hundreds of thousands of eggs in the egg warehouse. During the growth period, in addition to the regular discharge of menstruation, other eggs will also self-deplete as the years grow.

Without the aid of any medication, the female hormones secreted in the body are only enough to supply 1 to 2 eggs to mature, and the remaining small follicles that are not selected will shrink on their own. The ovulation needle only provides enough hormones to allow these eggs to grow up and be taken out, and will not affect the inventory or premature menopause.

  • Are there any risks in egg retrieval surgery?

The most worrying risk after receiving ovulation induction treatment is "Ovarian Hyperstimulation (OHSS)", the incidence rate is less than 5%. Our hospital designs the most appropriate ovulation induction treatment according to your ovarian function, and matches with sustained ovulation injections and different Egg-breaking injections to greatly reduce the probability of OHSS.

  • Do I need to get pregnant via IVF after freezing eggs?

Yes. The ovulation injection treatment is carried out, and the young eggs are surgically removed and stored in advance. Wait until the right time, thaw the egg for subsequent fertilization (IVF treatment), and at the same time incubate in vitro for 3~5 days before implanting the good embryo into the mother's uterus. The whole pregnancy will not be completed until the healthy baby is born after term.

  • Will using frozen eggs affect the subsequent fertilization rate and pregnancy?

We adopt the Japanese Kuwayama vitrification technology, and successfully completed the first case of egg freezing in November 2009. Every year, more female friends continue to use this method to successfully fulfill their wish of becoming a mother.

In addition, according to the latest statistical analysis of this fertility center, the survival rate of frozen eggs is above 90%, and there is no significant difference between the fertilization rate, clinical pregnancy rate and implantation rate of fresh eggs. Some males might suffer from infertility due to some diseases such as congenital chromosomal abnormalities and endocrine system abnormalities. Or testicular dysfunction, varicocele, sperm transportation dysfunction. etc. Or due to living habits, working environment, physical and mental illnesses and other factors such as drugs, radiation and sexual transmitted diseases, etc.
For these reasons, couples might need to borrow sperm form others to achieve their dream of having a complete family. 
Stork Fertility Centre has our own Sperm Bank, donors went through strict examinations and health assessments. As we have our bank, the waiting time for a suitable donor will be reduced to around 1 month after handing in all the required paper works.

· What health assessment did the donor have?
the donor will go through assessment including infectious diseases such as hepatitis B, hepatitis C, syphilis, gonorrhea and AIDS; Tests for genetic diseases such as thalassemia (complete blood count, CBC) and spinal muscular atrophy (genetic testing), karyotype testing, and Fragile X gene screening, etc. 

· Is it good to use thawed sperm? 
The donor's sperm preserve by the slow freezing method mixed with the cryoprotectant and gradually cooled down to -196 C. The survival rate after thawing is about 30-50%.
Preservation of sperm in extremely low-temperature liquid nitrogen equals freezing time, and storage for more than ten years is not a problem.

· How many times can we use the sperm?
It depends on what treatments you are undergoing. 
In Vitro Fertilization: approximately 3-4 times using Intracytoplasmic Sperm Injection, ICSI
Intrauterine insemination: approximately 1-2 times

· Can we see donor's photos?
No. Medical care institutions shall provide information concerning the donor's ethnicity, skin color, and blood type for the reference of the recipient couple only. We can not revive personal information about the donor due to the law restriction. 

· Can I choose a specific donor? 
According to Taiwan's Assisted Reproduction Act, "A medical care institution performing assisted reproduction shall not use a specific donor’s reproductive cells as requested by the recipient couple; a medical care institution shall not use donated reproductive cells for a specific recipient couple as requested by the donor.

· Do you have caucasian donors? 
No, all our donors are Taiwanese(Asian).
