Checkups for Infertility couples

Hysteroscopy (H-scopy)
Purpose:
H-scopy uses a hysteroscope to examine if the uterus and uterine lining are normal. There is a direct correlation between the condition of the uterus and embryo implantation.
Procedure:
Patient will be asked to lie on her back on the examination table with feet raised and supported by stirrups, similar to a pap smear or vaginal exam. An optical instrument connected to a video unit with a fiber optic light source is inserted to check the conditions of the uterus.
If only a simple examination is required, there is no need for anesthesia. The whole process takes about 3 mins, after which, the patient is asked to rest for another 30 mins. For further examination and operation, anesthesia will be administered and the process will take about 5 mins. Resting period is an additional 1-2 hours.

Best time to test is after the last menstrual cycle and before ovulation takes place for the next cycle. If it is a regular cycle of 28 days, the best time will be in the morning of the 7th to 11th day.

Chronic inflammation may cause endometirum polyp(s), tubal adhesion, obstruction and/ or hydrosalpinx, etc. Bacteria, Chlamydia trachomatis and human papillomavirus (HPV) are 3 most common pathogens.

Blood test for
couples’ chromosome
Based on
stats, over 70% of spontaneous abortion are related to abnormality of fetus’
chromosome. It is important to check for couples’ chromosome if you have any of
the following symptoms:
- Infertility with unknown reason, poor sperm quality, azoospermia
- Fail implantation with accumulated 4 GBR or 2 blastocysts with normal
chromosome
- Recurrent miscarriage or habitual abortion

For patients that cannot find sperm at multiple sperm analyses, doctor collect the first will collect urine sample after ejaculation, to eliminate the possibility of obstructive azoospermia. After that, blood tests are suggested to evaluate patient’s spermatogenesis.

[Patient Testimonial] I was sophomore, I chose freezing my eggs

As I was sophomore, I was diagnosed that I had a 13cm cysts. Two years later, I was diagnosed that I have several polyps. I did the surgery therapists twice with Doctor's suggestion. After that, I thought everything will be better if I keep healthy daily habits and diet, and following up with clinic. However, it wasn't easy as I thought, more unexpected gynecological conditions have been diagnosed.
I was told I might be infertility when I was 24. At once of following up, Doctor's told me seriously "you are retaining water in your fallopian tubes. The treatment for you is either block it or cut it off. You need to make a decision." Looking at the transferred tube from ultrasonic, I felt deep frustrated. I can't believe that it just happened on me. If I want to have a baby in the future, which means I couldn't be pregnant in the natural way. If I want a kid, the Only way is IVF? There were lots of questions in my mind. I could not stop worrying and crying. Afterawhile,I organized my thoughts and started facing on the issue. I knew I need to do something for my future. Beside seeing the doctor, next step is "having my egg to frozen".Therefore, I started searching and collecting the information from the hospitals and fertility centers. I also read some stories from the internet. After reviewing all the comments, I chose to freeze egg at Stork Fertility Center, and scheduled the first consultation.I still remembered the feeling of the moment I walked into clinic. I felt nervous and helpless; and hope everything would go through well. In order to do fertility assessment, I had the first appointment with Dr. Wang, I told him my medical history. On the third day of my period, I went
back to the center and took ultrasonic scanning. I told Dr. Wang, that I didn’t have any
confidence in my condition. However, I still remembered he encouraged me and
said “don’t worry and let’s work on it together.” I felt hopeful and believed no
matter how my treatment will be, Dr. Wang will be there for me. After seeing
Doctor, they arranged me to inject the first long-acting rFSH and needed to
take some luteal support in following days. Donna explained all the details
after then. My feeling was complicated, but happy for starting my first step of
my treatment. Additionally, the APP of from the Stork showing all the steps of
my treatment and the data is really convenient for me to track the status.
However, the app doesn't have English version right now. I believe it will come
out soon.
During the 9th
day of the period, I took the ultrasonic scanning again and then taking blood
testing. Dr. Wang checked my test result and had positive comment on it. He had
confidence to collect at least 10 eggs. Therefore, in order prompt my body to
produce multiple egg, I would be prescribed injection medications.
On 14th day of
my period, it's the day for eggs retrieval. I woke up early and went to Stork
for checking in. I was guided to surgery room. I was checked with ultrasound.
The Sonographer told me "Did you see? You have many eggs in your belly
now." I felt relax and worthy. After surgery, I felt pain. But I heard we
had a great job from Dr. Wang. The surgery was successful and had a great
result. There were total 28 eggs with this retrieval that
was better than I expected. However, I got the bad news about my cysts were
back again. By freezing egg, I felt much power and courage to endure all myhealthconditions.I’m
glad I save my wishes in Stork Fertility Center. This was my biggest gift of my
24th birthday.
I went home and ate the food
that the center provided. While I was guessing how much eggs would be storaged
this time, and I got the call from them. They told me there were 20 eggs were
mature that could be frozen. I looked the
medicine what I brought from clinic, I knew it will be done after I finished
all the medicine. The detail information of my eggs
was shown on the APP I felt surprised again, due to I saw the details of
my eggs and the photos. It was to wonderful.
One day, I will get married
and have kids. I don't want anything stop my future life. I hope my story would
encourage those girls who face the same problem as me. Please don't ignore the body
signals as you are young. Don’t hesitate, believe yourself, be brave, no

Do you know what eggs collection is?
Say simply, it is collecting good quality oocytes to freeze them. When the number of good oocytes is collected a certain number, starting to thaw these oocytes and then fertilize them.
What kind of situation that women need egg collection?
Let’s me tell you. We all know that ovarian function depends on women’s’ age. As the ages grow, ovarian function begins to decline. Also, the quality of oocytes is gradually getting worse.
Then how do we know our ovarian situation?
There is one important index called Anti-Mullerian hormone (AMH) which is produced by small growing follicles and granulosa cells in ovary we can refer to.
Based on this index, we learn that AMH can be represented as the ovarian situation.
As result, on condition that our AMH is low, we have to seize the chance to collect the good quality oocytes while our ages are not too old (under 38 years old).
*If our age is more than 38 years old, that is completely another thing. What We should do is Blastocyst Collection, not egg collection.

Besides AMH, age can also convey the important sign to us. How many mature oocytes are enough for women to have a baby?

In view of this index, we should go further to know how to distinguish which oocytes are mature.
At first, we judge the cumulus-oocyte complex initially from I to III. Then by using enzyme to remove the cumulus, we can evaluate oocyte morphology and choose the oocytes which are in stage MII to frozen it.
*Only oocytes which are in stage MII is valuable and can be fertilized.
For whom is under 38 years old, the best way to enhance the pregnancy rate effective is egg collection.
According to these two indexes which is related to individual ovarian function, we will apply the individualized controlled ovarian stimulation (iCOS) to design the fittest protocol for each customer.
Besides the individually tailored treatment, we have the most simple and effective process for a customer who far from overseas.
The first time you come to Taiwan is for filling the relevant document and having a necessary examination such as hysterosalpingogram (HSG), and blood drawn. When the treatment starts, what you should do is having blood drawn and taking ultrasound on a specific day in your country for checking the development of follicular.

Traps from delicious foods? Eat right and have better sperm

Do you
like wearing a tight pants or putting your laptop on your thighs? These habits
may affect the quality of your sperm. Besides, five types of drinks/foods may
harm your reproductive system. Let’s check and keep in mind!
According
to <<Epicurious>>, if the
quality of your sperm is not good, it may affect your family plan. To maintain the good quality of sperm, the
uptakes of following drinks/foods must be controlled.
Alcohols dehydrates the water in our bodies, and increases the risk of
obesity, and reduces the mobility of sperms. We suggest not drinking alcohol
more than once per week, and therefore the bodies can metabolize the alcohol. But
if you have severe reproductive problem, it is better to quit alcohol.
2. Processed meat products
Bacon, ham and burger are delicious, but these processed meat products
usually contain high proportion of saturated fat and hormone residues, which
harm the quality of sperm. If you like meat, have the “fresh” one.
3. Sweet Drinks
Bubble milk tea or other sweet drinks contain high proportion of sugar
stimulating pancreas to release more insulin, and they increases the risk
of obesity. Sweet drinks reduce both the quantity and quality of sperm.
It’s better for us to drink water.
4. High-fat dairy products
Milk, cheese and other dairy products can provide protein, calcium and
other nutrients. But the high fat contained in these products may harm male’s reproductive
system. Also, pesticide or hormone residues should be concerned in some dairy
products.
5. Fruits with pesticide residues
Some of the fruits or vegetables may have pesticide residues, especially
apples, grapes, strawberries, spinach, cucumbers, etc. We suggest choosing
organic fruits and vegetables to decrease the risk.
Stay away from bad habits and unhealthy
foods/drinks. Get your sperm good!
