The Procedure of Sperm Recipient Program (Overseas)

What you should prepare first before coming to Taiwan.

1. Registration Form (please e-mail to our consultants. see contact us. Or register online)

2. Marriage Certificate (Based on your marriage registration places, let your marriage certificate get authenticated by TECO which in your marriage registration places.)

**Only the marriage certificate get authenticated will be regarded as valid in Taiwan.

3. Couple's photo ID and valid passport

4. Couple’s HIV, syphilis, and blood typing report

5. Other fertility medical records

6. Kinship form (Our government will check whether your wife and sperm donor are relatives, please record you and your wife relatives English name)

First Visit to Taiwan

**Mainly for concerning examination and sperm freezing, staying for only 1 days during MC Day 7-13

I. Meeting with the Doctor.

II. Complete and notarize Sperm Donation Program consent, IVF consent, Kinship Form and regarding documents.

***Two identifications of recipient couple are required (ID card and Passport)

III. Recommended Examinations as below.

· Wife's Examination

1. HSG (hysterosalpingogram)

2. Blood test of Autoimmune and Hormone

IV. Decide whether PGS (Preimplantation Genetic Screening) or not

V. Discuss your IVF treatment with the Doctor and when to start ovarian stimulation.

VI. Gain the personal IVF protocol and bring the medication back home.

Second Visit to Taiwan

***Mainly for follicle growth monitoring and eggs retrieval surgery, staying for 5-7 days during MC Day 7-9

I. Follicle growth monitoring

II. Eggs retrieval surgery

III. Wife's Examination

1. Hysteroscopy

2. Cervix Culture Test

IV. Eggs freezing (Eggs collection)

V. Nsemination and Blastocyst culture (For Embryos Collection)

Third Visit to Taiwan

***Mainly for transfer, staying for 3-5 days during MC Day 18-20

Fresh embryo transfer cycle

Synchronize the mense cycle with the donor by taking hormone pills (2 to 3 months required).

l Donor starts the stimulation procedure, and recipient starts taking pills for endometrium preparation.

(If you used our oocyte donor bank, the donor has completed oocyte retrieval.)

l Recipient follows up endometrium thickness by ultrasound.

l Stork Fertility Center will give notification if the date for donor’s egg retrieval is determined. (2 days before the egg retrieval)

l On the date of egg retrieval, husband collects sperm sample in Stork Fertility Center. (Or using the frozen sperm for fertilization).

l Fertilization takes place, embryo culture to day5-6.

l Fresh embryo transfer, around TWD 50,000-60,000 per cycle.

Frozen embryo transfer cycle

l Donor starts the stimulation procedure. (If you used our oocyte donor bank, the donor has completed oocyte retrieval.)

l Stork Fertility Center will give notification if the date for donor’s egg retrieval is determined. (2 days before the egg retrieval)

l On the date of egg retrieval, husband collects sperm sample in Stork Fertility Center. (Or using the frozen sperm for fertilization).

l Fertilization takes place, embryo culture to day5. And freeze all the resulting embryos.

l Frozen embryo transfer will be arranged according to recipient’s next cycle, around TWD 50,000-60,000 per cycle.
